Fitness Center Key Tag
A Key-tag will be distributed to each KSC Fitness Center member. On the back is a barcode and a unique membership ID. Use it as a quick and easy way to check-in at the front desk.
To obtain a Key-tag, please print and complete the form(s) which apply to you, then bring them with you when you visit the KSC Fitness Center. A trainer will be happy to process the forms.
- KSC Fitness Center: Demographics Information form, KSC Form 50-85
- KSC Fitness Center: Fitness Center Clearance Questionnaire form, KSC Form 50-42
The forms can be found online at the NASA Forms website, https://nef.nasa.gov, or picked-up at the KSC Fitness Center. On the NASA Forms website, search by the form number, such as "50-85" or "50-42". In the search results, click on the form number link on the left-hand side of the screen. Please do not click on the "order copies" button.
