Membership Information

An integral arm of the KSC Health Education and Wellness Program (HEWP), https://hewp.ksc.nasa.gov , the KSC Fitness Center (FC) offers complimentary membership to all employees with a KSC or CCSF badge. Required membership registration can be completed in-person during operating hours or you can bring the completed forms with you to the Fitness Center.

Required Membership Forms

The KSC Fitness Center (FC) is located in the northeast corner of the Neil Armstrong Operations & Checkout Building (O&C) , M7-0355,Room 1514. Enter the O&C building through the double doors on the East side of the building. The FC entrance will be directly in front of you.

1. Read NEMCON form OCH-F-0015-A, Kennedy Space Center Fitness Center Orientation Packet
2. Download and sign NEMCON form OCH-F-0015-B, Receipt and Understanding of Fitness Center Orientation Packet
3. Download, complete, and sign KSC Form 50-85, Fitness Center Demographic Information
4. Download, complete, and sign KSC Form 50-42, Fitness Center Clearance Questionnaire (FCCQ)
   • The Fitness staff will check your blood pressure at the Fitness Center, as part of the membership registration. IF you answer “yes” to any of the questions on form 50-42 (FCCQ), and/or if your blood pressure is 140/90 or above, you will need an additional Medical Clearance from either your physician, or the Occupational Health Facility (OHF).
